Our care and support at home service is designed for individuals who need assistance with daily activities but prefer to remain in their own home. 

We offer a variety of support options, including help with personal care, household chores, and companionship, to ensure that you can maintain your independence and quality of life.

  • Personal Care: Assistance with bathing, dressing, grooming, and other aspects of personal hygiene.

  • Household Support: Light cleaning, laundry, and tidying up to maintain a safe and comfortable living environment.

  • Meal Preparation: Nutritious meal planning and cooking based on dietary preferences or health requirements.

  • Companionship: Engaging in conversation, playing games, and going out for walks to promote mental well-being and social interaction.

  • Medication Reminders: Ensuring that you take your medications as prescribed.

  • Mobility Assistance: Support with getting in and out of bed, using walking aids, or moving around the home.

Care can be provided on a part-time or full-time basis, with support available when you need it most, this makes it a more flexible and affordable option which is not disproportionate to your needs. Our Assessment will ensure that your care plan fits around you, celebrates and emphasizes what you can do for yourself and compliments the way you do things. 

How it works

1. Consultation: We meet with you to discuss your care needs and develop a plan tailored to your requirements. We’ll talk to you about how our service operates, and who you might see most often. Wherever possible we will match you with someone local or someone who shares your interests

2. Assessment: We assess your daily activities, health conditions, and preferences to create a customised care plan. We use a digital care planning system which you can access at any time. Not keen on tech? No problem, we can also give your loved ones access or share printed copies with you. 

3. Care Delivery: We will introduce you to your regular carers and they will support you as per your personal care plan.

4. Ongoing Reviews: Regular reviews ensure that your care plan remains relevant and your needs are consistently met. We also use this time to check in with you and make sure you are completely happy with your care.
